A high degree of collaboration and trust enables
the Metro North Hospital and Health Service and the Metro North Brisbane
Medicare Local to improve the local health system across a shared geographic
catchment of over 855,000 community members. The presentation will
highlight collaborative programs that are keeping people well and out of
hospital by better integrating primary and acute health care services. A
number of ‘Australian-firsts’, including Team Care Coordination, Map of
Medicine and Home and Community Care, demonstrate the innovative responses
initiated locally which involve GPs, community providers, NGOs and hospital
clinicians all working together to create a community where good health is
available to all. The shared governance and planning arrangements which
underpin these efforts and which are enshrined in a Protocol between the two
organisations will also be discussed.
